Community Health Systems Integration Clerkship
This four-week Phase II clerkship offers medical students' meaningful exposure to diverse community-based care settings within Penn State Health’s integrated academic health system. Clinical experiences span physician offices, urgent and emergency care clinics, outpatient surgical centers, and specialty clinics. Through these rotations, students will strengthen their history-taking, physical examination, assessment, and care planning skills while learning to navigate patient care across settings. Emphasizing whole-person care and health science–driven decision-making, the clerkship introduces students to value-based care principles and the importance of working collaboratively with interprofessional teams to optimize outcomes for patients and families.
Assess and enhance the ability to provide whole-person, community-based care to both healthy and medically complex patients across Penn State Health, while applying principles of health systems thinking to improve care coordination, optimize resource utilization, and drive better population health outcomes. This is a non-specialty-specific rotation designed to foster understanding of how individual care decisions intersect with organizational processes and system-level goals.
